NW Konnection Services provides support and transition services to youth and adults who experience an intellectual and/or developmental disability who qualify with ODDS - plan and live in their family home or independently in the community. We serve individuals living in Clackamas, Clatsop, Coos, Hood River, Lane, Lincoln, Malheur, Marion, Multnomah, Tillamook, Wasco and Washington counties in Oregon.

NW Konnection Services works with county and brokerage case management entities to provide customized service in meeting your ISP desired outcome goals.
